Watch and Sign Waivers

All participants and visitors entering the climbing area of the facility must have a waiver on file. Minors must have a waiver signed by their parent or legal guardian.

INSTRUCTIONS FOR PARENTS: 
ADULT NAME -and- ADULT BIRTHDAY must be entered first.
Child's information will be entered later

IMPORTANT!
1) Waivers for minors must be signed by a PARENT (or COURT APPOINTED legal guardian).
2) Waivers signed by a friend, relative or group leader WILL NOT BE ACCEPTED.
